ABOUT PATHWAYS

Pathways is one of the largest providers of community based behavioral and social services in America. In 17 states and the District of Columbia, more than 5,000 dedicated Pathways employees create healthier communities by addressing social determinants of health and providing home and community-based services.

Pathways was founded in 1997 and provides direct services and case management for clients of all ages, including Youth & Family Services, Adult Services, and Prevention Services.

Healthcare providers are today focused, more than ever, on delivery of patient-centered care that results in optimal outcomes, reduced re-admissions and lower costs. Pathways provides a focused approach to managing the medical, behavioral and social needs of potentially high-cost healthcare consumers with chronic conditions and complex social challenges. With over 20 years of experience, Pathways is uniquely positioned to address and lower the additional healthcare costs for those who suffer from medical as well as behavioral and social determinants of health challenges.
