Pathways of Washington was founded in 2014 with the dream of bringing exceptional behavioral health directly into the homes of disadvantaged populations. Our services began in Adult Family Homes and Assisted Living Facilities, where we worked with residents to maintain their current living situation through behavioral intervention. Fundamental to this work was supporting in home staff with on the job education, skills building, and trainings in order to strengthen the therapeutic relationship with their residents. This program has stabilized hundreds of clients in their home setting since that time, reducing community homelessness, and building lasting human relationships between provider and clients.

Back then, Pathways was only in Spokane County. Since that time our service model has exploded and we now offer high quality, in home care to counties all across the state.

Since its inception, Pathways of Washington has grown, expanding its mission to bring exceptional social services into the heart of communities across the state to address some of the gravest issues facing us today.

Our Housing and Employment programs assist Medicaid eligible individuals by pairing them with a 1 on 1 subject matter specialist. These specialists work with clients, providing support and expertise while working towards placement.

Our Health Homes Care Coordination program works with clients across the state to improve their access to, and utilization of, the healthcare services in their community. Pairing together people and community supports to improve their quality of life and producing improved outcomes.
